How to Find Your NHS Number
An NHS number is a 10 digit number, like 485 777 3456.
Your NHS number is unique to you. It helps healthcare staff and service providers identify you correctly and match your details to your health records.

Register with Mansion House Surgery
Register Online
Step 1: Check Catchment Area
Use the map (see above) to check if you live within our catchment area, if after checking you are still unsure please contact reception to confirm.
Step 2: Complete a Patient Registration Form
Our online registration form will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.
NOTE: This surgery is based in Staffordshire, we are aware of a practice of the same name based in Whitehaven, Cumbria.
Step 3: Register for Online GP Services and Upload ID
Please register for Access to Online GP Services. To register for online services we do require a copy of your photo identification (passport, driving license or EU Identity card). You can upload a copy of your identity with the 'Upload ID' button below, or if you prefer to submit your documents in person please speak to the reception team.
Register using the NHS Website
Go to the Register with a GP Online Service (opens in a new browser window) and follow the on screen instructions.
I do not want to register online
If you prefer not to register online please complete the following steps:
- Use our Practice Catchment Area Tool or check the map above to ensure you live with our catchment area.
- Download and complete a Register with a GP Surgery (PRF1) form (also available from reception).
- Return your completed registration forms to reception along with your photo identification.
Temporary Residents
The Doctors are happy to see patients who are temporarily resident in the Practice Catchment area should medical advice be required. In this instance persons wishing to be treated should state that they are a temporary resident when reporting to reception.
To register you will need to bring your passport and complete our Temporary Resident Registration (GMS3) form (also available from reception).
Valid forms of Identification
To register we require photographic identification (ID), for example passport, drivers licence or European Union Identity card.
If you are homeless or of no fixed abode, then although it is helpful to have photo ID you can register without any.
Any other ID such as an HC2 Certificate, Rough Sleepers' Identity badge, Hostel registration or mail forwarding letter would also be helpful but again we will register you even if you don't have these. As we know it is more important that you can access health care when you need it.
If you are unsure on how to register, please ask at reception and they can help you to complete the registration process.
Accountable Named GP for All Patients
All patients registered at the Mansion House Surgery have a named doctor who has overall responsibility for your care and support. Your registered GP is also your named accountable and your allocated GP. Your allocated GP will be responsible for the provision of your healthcare. If you choose to see another doctor at the surgery you are entirely free to go on doing so exactly as before.
If you are unsure of who is your named Doctor please ask at reception.
New patients are allocated a Doctor on Registration.
